Abstract
La causa de la calvicie masculina radica en la sensibilidad de los folículos pilosos hacia la dihidrotestosterona. La enzima 5-alfa-reductasa convierte la hormona testosterona en dihidrotestosterona, y los folículos responden a ella reduciendo su tamaño, hasta que finalmente, las membranas del folículo piloso se tornan rígidas. De esta forma, la estructura folicular recibe menos irrigación sanguínea y los cabellos nuevos son más débiles y finos de lo normal. Al final del proceso, los folículos se atrofian y el cabello que se cae no es reemplazado por cabello nuevo.
Se han estudiado los niveles de dihidrotestosterona en personas con alopecia androgenética y suelen ser normales. Por lo tanto, se ha determinado que no es un nivel elevado de dihidrotestosterona lo que produce la alopecia, sino una sensibilidad particular de los folículos frente a la dihidrotestosterona.
